Required forms for the start of school
The following forms are required within the first week of school:
Student Health and Information Sheet- Please fill one form PER CHILD and submit within the first week of school.
Currituck County Library Card Application. New students will need to submit one of these forms before the initial library class on the first Wednesday of the academic year.
Rising Kindergarteners and 7th graders require immunization records within the first 30 days of school. See the immunization section of the handbook. Your doctor can fax records to 252-453-3154.
Educational Leave of Absence Form. If you plan to pull your child from school for an experience that will provide educational value, please submit this to your student's homeroom teacher at least one week in advance for absences of less than five school days, or to the Board of Directors at least two weeks in advance for absences of five school days or more.
